Created Your Personal Portal. Share your personal health story, including your main concerns, what you have already tried, and the questions you would ask me if we were sitting together in my office.

Complete the requested health form.

Include your current medications and supplements, including the product name, dosage, and how often you take them whenever possible.

All of your personal health information will be handled through my HIPAA-protected patient portal, keeping your health history, laboratory results, recommendations, and communication together in your confidential patient record.

2. UPLOAD YOUR LABORATORY RESULTS AND SUBMIT

Upload any recent routine bloodwork or functional medicine laboratory testing you would like me to review.

You do not need to decide which individual results are important. I will review your information together as one complete picture.

Once everything is included, simply submit it through your secure patient portal.

3. RECEIVE YOUR PERSONALIZED RECOMMENDATIONS

I will personally review your health history, concerns, laboratory results, medications, supplements, and questions from a functional medicine perspective.

Depending upon your individual situation, your recommendations may include:
